If you have medical or dental insurance you would like us to bill, you must provide the information several days prior to your appointment. An exam prior to treatment is highly recommended as many insurance companies require preauthorization or predetermination of treatment several weeks prior to services being rendered.  Please bring a copy of your insurance cards with you to the consultation so that we can ensure correct billing of your claims.
